How to embed Google Form in Outlook email
You have just created a Google form. Now you want to send the form email. How to send the form embedded in the email, using Outlook?
# In Google Form, Press the "Send" button

Click on the email tab and then fill your own email (that you access using Outlook)
# Send the form to yourself
Select the "include form in email" option 
Then press the send button.
# Forward the email from outlook
You may want to format the top section. Then send the email from Outlook.
